DAN Services

New Castle County - Care Management Services

Jewish Family Services provides DAN Care Management services in New Castle County. Alexis Morris assists seniors and their caregivers by providing information about Medicare, Medicaid, housing, health care, in-home services, transportation and other issues.

Modern Maturity Center provides DAN Care Management Services in Kent County. Cheryl Gallagher assists seniors and their caregivers by providing information about housing, health care, in-home services, transportation and other issues.

If you are interested in Care Management services please call the Modern Maturity Center at (302) 734-1200, Ext. 173. Services are available Monday through Friday from 9:00 am – 5:00 pm or drop in to see all that is happening at Modern Maturity Center.

Sussex County – Transportation Services

The DAN Sussex County Mobility Consortium managed by CHEER has devised a coordinated system of transportation for older and disabled adults. Through a network of volunteers, the public transportation system, and agency buses, older and disabled adults are receiving needed transportation.
