The Delaware Aging Network

is an Aging-in-Place Initiative for older adults.

 

Beginning with the vision of four agencies working in support of older adults,  DAN quickly evolved into a consortium of agencies focused upon serving older adults. With initial funding from the Delaware Community  Foundation,  the consortium now consists of more than 50 agencies across the entire state of Delaware committed to improving the quality of services older adults receive.   In 2009, DAN expanded its care management services in New Castle and Kent Counties to Sussex County. In addition to providing and coordinating services for seniors, DAN advocates for state-wide policies  benefiting the aging population.

 DAN helps older adults continue to live in their own homes as they age and need more support.  DAN care managers work directly with seniors through many local senior centers. 

Care managers are eager to provide you the information you need regarding:

     ● Medicare
     ● Medicaid
     ● In–home services
     ● Transportation
     ● Other community resources
