Care Management Services

DAN Care Management Services

DAN care managers work directly with adults ages 50 and over through the senior centers listed below. Care managers provide information about housing, health care, Medicare, Medicaid, in – home services, transportation, and other community resources.

Referrals are made to agencies to meet the personalized needs of each individual. If you need assistance, call DAN in your county or visit one of the senior centers. Click on the counties below for more details about each county.

New Castle County -
Care Management Services

Jewish Family Services provides DAN Care Management services in New Castle County. Alexis Morris assists seniors and their caregivers by providing information about Medicare, Medicaid, housing, health care, in-home services, transportation and other issues.

If you are interested in Care Management services please call Jewish Family Services at (302) 478-9411, Ext. 126 or by e-mail at amorris@jfsdelaware.org. If you live near a senior center, we can usually arrange to meet with you there for your convenience.

Modern Maturity Center provides DAN Care Management Services in Kent County. Cheryl Gallagher assists seniors and their caregivers by providing information about housing, health care, in-home services, transportation and other issues.

If you are interested in Care Management services please call the Modern Maturity Center at (302) 734-1200, Ext. 173. Services are available Monday through Friday from 9:00 am – 5:00 pm or drop in to see all that is happening at Modern Maturity Center.

Sussex County –
Care Management Services

CHEER provides DAN Care Management Services in Sussex County. Cindy Mitchell assists seniors and their caregivers by providing information about housing, health care, in-home services, transportation and other issues.

If you are interested in Care Management services please call CHEER at (302) 856-5187. Services are available Monday through Friday from 8:30 am – 4:30 pm and is available additional hours and days by scheduling an appointment. Drop in to see all that is happening at CHEER.
