Care Management

Care Management focuses upon meeting the multifaceted and specialized needs of older adults. Its confidential, caring and professional services are coordinated through Jewish Family Services in New Castle County and the Modern Maturity Center in Kent County. The services address the physical, mental and emotional challenges of the older adult with the hope they will age-in-place safely with appropriate support services. All services are provided in senior centers throughout the counties or in homes. Available services include:

In-Home Services

    Case management

Legal Support

    Referral to attorneys

    Assistance in completing applications & forms (Power of

    Attorney & Living Will documents)

Medical Needs

    Medicare counseling

    Referrals to medical personnel

Transportation

    Medical transportation for older adults through Generations Home Care

Fees may be associated with some of these services.

Additional services are offered at Jewish Family Services including:

GEM (Growth Education Mental Fitness)-a series of programs combining learning & community building with a healthy & mental and physical focus
Housekeeping services to promote safety and hygiene
Out-patient mental health counseling

Additional services are offered at Modern Maturity center including:

Telephone reassurance program through Modern Maturity Center's Are You Okay program
Access to food from the Food Bank's Free Food Box Give Away, the Whatcoat United Methodist Church 's monthly food distribution and for a modest cost, Angel Food Ministries
