About Us

 VISION

To provide a holistic system of care for older adults assuring safety, well being and independence

PURPOSE

Older adults in Delaware want to remain independent as they age, living in their own homes with access to services and activities in the community that sustain and enrich. The Delaware Aging Network (DAN), with initial pilot project funding from the Delaware Community Foundation, will create a system of care to help seniors meet the challenges of growing older. The foundation of this system will be an integrated, collaborative network of service providers that will expand outreach to and services for the aging population, while addressing the interrelated concerns of housing, health and transportation. This statewide coalition of nonprofit organizations, working through lead agencies in Delaware’s three counties, will increase efficiency, reduce duplication of services, and assure that more seniors receive the care they need in order to remain independent.

OBJECTIVE

To develop and maintain an inclusive, accessible and collaborative system of information, referral, and service delivery to maximize the physical and emotional well being of Delaware’s older adults

GOALS

  • Improve older adults’ abilities to access necessary support and referrals.
  • Prevent isolation and enhance quality of life for older adults through improved mobility.

  • Enhance collaboration between senior service agencies in each county and statewide.

  • Expand the service capacity of network agencies and reduce duplication of services.
  • Develop long-term project sustainability.
